Program Overview

The consortium, comprising Customized Medical Needs, Customized Veterans Group, and BLAC PAC, aims to tackle employment, housing, and healthcare issues for vulnerable populations, focusing on financial and mental health.

BLAC PAC is a 527-member nonpartisan nonprofit organization that fosters leadership and community development across all racial and ethnic communities. Upholding the principles of Excellence, Integrity, and Discipline, BLAC PAC is committed to creating long-term solutions that uplift society’s most vulnerable.

CVG, a Tennessee-based 501 (c)(3) nonprofit, focuses on supporting veterans in Shelby and Tipton Counties. CVG’s research highlights prolonged healthcare wait times, homelessness, and mental health issues as the key challenges facing veterans.

CMN provides rehabilitative therapies and mental health services to veterans, civilians, and individuals in underserved communities. CMN has expanded its services to address substance use disorders and mental health conditions, recognizing the critical need for integrated healthcare.

Homes of Honor is committed to ending veteran homelessness by providing lifelong resources, including stable housing, healthcare, and community support. We also support workforce development, offering training and employment opportunities to enhance veterans’ long-term wellbeing and success.

Homes of Honor provides affordable, temporary, and long-term housing to veterans, promoting health, independence, career growth, community sense, reduced drug addiction, and improved crime rates.

Stable housing benefits veterans’ physical and mental health, enabling better healthcare access, better chronic condition management, and reduced emergency room visits and long-term healthcare costs.

Promoting Independence & Career Growth

Housing stability improves independence, reduces stress, and allows veterans to focus on career growth and education, enhancing their sense of purpose and self-worth.

Sense of Community and Support Networks

Affordable housing reduces social isolation among veterans, promoting community participation, relationships, and peer support. This can lead to mental health recovery and increased life satisfaction.

Lowering Drug Addiction Rates

Stable housing and rehabilitation services significantly reduce substance abuse among veterans, with a 25% higher recovery rate reported by the National Coalition for Homeless Veterans.

Reducing Crime Rates

Homes of Honor provides affordable housing to veterans and reduces crime rates by up to 40%. This holistic approach includes mental health support, life skills training, and career development.

Since 2013, CMN has supported 1,013 veterans through various healthcare services. We expect this number to rise, driven by the aging veteran population and the increasing prevalence of chronic conditions.

In 2013, 2% of Shelby, Fayette, and Tipton Counties veterans required skilled nursing care, with an expected annual increase due to aging.

Occupational Therapy (OT) and Physical Therapy (PT)

Registered Dietitian Services

Diet-related conditions, such as diabetes and cardiovascular disease, have driven 3-5% of CMN’s veteran clients to seek dietitian services.

Veterans with neurological or injury-related speech impairments have accounted for 1-2% of CMN’s census, requiring speech therapy services.
